Certification of Clinical Laboratory Physicians

Certification of a Certified
Clinical Laboratory Physician will be implemented according to the
regulation set by the Japan Society of Clinical Pathology (JSCP), as
described below.
-
Eligibility requirements for the
certification examination are as follows:
1) Applicant must have a medical practice license issued
in
Japan and must possess personality, principles, and
values
appropriate for a medical doctor.
2) Applicant must be a member of the JSCP for five or more
consecutive years at the time of application.
3) Applicant must have successfully completed a five-year
training program for a laboratory physician designated by
the JSCP.
However, JSCP membership and training
requirements will be decided
separately for applicants in
the following circumstances: a
Certified Physician in
any of the basic clinical areas
specified by the
Association of Certified Physicians, who began the
training program with an intention to become a Certified
Clinical
Laboratory Physician, those with Clinical
Pathologist
certification from the US, or equivalent
qualification from other
nations.
4) Applicants must have completed at least two years of
training program at JSCP accredited training centers at
the time of
application. Required training programs
include all of, but
not limited to, the following:
a) General Clinical Pathology
b) Urinalysis and Clinical Microscopy
c) Clinical Hematology
d) Clinical Chemistry
e) Clinical Microbiology (including Infectious Diseases)
f) Clinical Immunology
g) Blood Banking
5) Applicant must have completed one or more years of the
following academic program at the institutions certified
by the Ministry of Health and Welfare. The
academic
program may be any one of the following:
a) Anatomic Pathology
b) Clinical Medicine (this should be part of the
first-year
post-graduate training program designated
by the Certified Physicians Association's in the
basic clinical area or part of comprehensive general
practice.
However, an applicant with one of the following
qualifications will be exempted from training and
examination for the elective
subjects:
the Certified Physician in any of the basic clinical
medicine specified by the Association of Certified
Physicians, who began the training program with an
intention to become a clinical laboratory physician
and the applicant with Clinical Pathologist or
Clinical Laboratory Physician certification from the
US, or equivalent certification from other nations.
6) Applicant must submit a total of 20 reports to certify
their daily routine work at the clinical laboratory. The
reports can be on a variety of topics including, but not
limited to, consultation, bone marrow examination ,
immunoelectrophoresis, chromosome analysis, and other
laboratory interpretation , and On-Call conference
records. However, the reports related to surgical
pathology and summary records of patients which are
considered to be part of internal medicine should not be
included.
7) Applicant must be the first author in three or more
papers or reports related to clinical pathology submitted
to the JSCP. As a general rule, it is preferable that
those reports or papers be presented at the JSCP
scientific meetints during five-year training period.
8) Applicant must be recommended for membership by a
supervisor of his/her training.

The judgement of eligibility for
certification examination and its certification examination will be carried out based on responsibility and standard by the JSCP.

The issurance of the certificate and details of the certification examination will be designated separately.

The expiration date of certificate for the Certified Clinical Laboratory Physicians shall be five years. Recertification will be conducted every five years.
Additional rules
-
This regulation will be effective January 1, 2000.
-
For the time being, training centers designated by the JSCP in the above number 4) of section 2 shall include its equivalent ones.
Qualification for examination of those who have studied at other training centers
described in the above numbers 4) and 5) of section 2 shall be designated separately and applications for the examinations can be approved by examining their training records.

Board of Directors will approve the revision of this regulation.
Regulation for Recertification

The JSCP shall implement the recertification according to the following procedures in order to maintain a level of the Certified Clinical Laboratory Physician(s).

Those who wish to apply for the recertification must have been members of the JSCP continuously, after certification, until the next application.

The Committee on Qualification for
Examination/Recertification the Commission on Certification of Clinical Laboratory Physiciand /JSCP will manage the
recertification.
-
The recertification shall be approved to those who have attended educational programs within five years after certification, which are specified by the Commission on Clinical Laboratory Physicians of JSCP, and have acquired credits as follows.
(1) There must be total credit of more than 50 points.
(2) At least 25 out of 50 credits must be acquired through
the programs planned by the JSCP(Classified as A in the
below credit table).
(3) The 25 credits described in (2) shall include at least
one credit through attending either the (National)
General Congress or the Special Conference of the JSCP.

The number of credits obtained by planning and/or participating, which are required for the recertification, will be calculated as shown in the following credit table.
(1) Credits described as A in the following table will be
acquired only by the first author making a presentation
or a report at the JSCP.
This credit shall be calculated only one topic per
conference.
(2) The other affiliated societies described as C, refer to
lectures and educational seminars held by scientific
organizations, including The Japanese Society of
Internal Medicine, The Japanese Society of Pathology,
The Japan Society of Clinical Chemistry, The Japanese
Society of Hematology, The Japanese Society of Clinical
Hematology, The Japanese Association for Infectious
Diseases, The Japanese Society of Chemotherapy, The
Japanese Society for Clinical Microbiology, The Japan
Society of Blood Transfusion, The Japan Society for
Clinical Laboratory Automation, Japan Society of Medical
Electronics and Biological Engineering, The Japan
Society of Ultrasonics in Medicine, and Japan
Association for Medical Informatics.
(3) The first author or co-author of academic papers in the
journal of the JSCP, the Japanese Journal of Clinical
Pathology, will gain specified credits respectively as
described in D in the following credit table.
(number of credits in parenthesis is for co-author.)
Only the first author will gain specified credits, in
case of academic papers for other scientific journals.
The journal should have a referee system and papers
should refer to the clinical pathology.
An author name,
a subject, a name of journal, a volume and a published
year must be written in this order on the treatises.
(4) It is desirable for those who receive the first
recertification after the year of 2003 to include a
report of routine work (20 volumes), in the required
credits (described as E in the following credit table).
However, it shall exclude the everyday anatomic
pathology reports and
consultation records for internal medicine (ex.
hospitalization summary records, etc.).

Registration of credits for the recertification must be prepared in a specified way, when he/she attends the lecture sponsored by JSCP and JACLaP
Except for those sponsored by JSCP and JACLaP, a documentation to prove attendance of congress or lecture (for example a receipt for attendance fee, a certification of participation or a copy of it) must be attached.
Procedures of applying for credits by the participation will be instructed separately.
When a Certified Clinical Laboratory Physician reaches the age of sixty-five, he/she may renew a qualification by another application procedure designated separately, if he/she is a member of the JSCP continuously.

If the number of credits required during the specified term is insufficient for recertification , an applicant may apply for a reservation of the recertification . An applicant may re-apply after taking the required number of credits at a later date. The term of the reservation will be a maximum of two years. An applicant may not use for oneself the name of the Certified Clinical Laboratory Physician by JSCP during these terms due to invalidation of certificate.
However, the applicant may apply for an extension of a reservation attached with valid reasons in the case of studying abroad, illness, or unavoidable circumstances. The applicant is allowed to extend the reservation term for up to five years after disqualification, if the Committee on Qualification for Examination/Renewal approves.

An applicant shall regain the qualification for the recertification l, if he/she acquires the necessary credits, takes an examination for the general clinical pathology described separately, and passes within three years, even after expiring the reservation term.
Additional rules

This regulation will be effective, starting April 1, 2000.

Regulation of Certified Training Facilities

Training facilities designated/certified by the JSCP include the following:
1) JSCP-certified training hospitals, JSC-precertified
training hospitals, and special fascilities designated by
JSCP.
2) Facilities abroad where equivalent or higher level of
training can be conducted to the above.

The JSCP certified hospitals P must provide the programs sufficient for trainees to experience and train for themselves on the subjects designated in the post-graduate clinical pathology training curricula, satisfying the following conditions .
1) The hospitals must have at least 300 beds, or the
equivalent size.
2) A full time Clinical Laboratory Physician certified by
the JSCP must be in charge of education.
3) An educational curriculum for post-graduate clinical
pathology training must be provided n for more than
2-year period related to the seven required subjects
established by JSCP.
4) Clinical laboratory shall be centralized functionally.
5) There must be personnel, facilities, medical instruments,
and library, sufficient for clinical diagnostics,
research , and education, their. results must be reported
in the JSCP meetings.
6) The committees on quality control and appropriate
laboratory utilization and clinico-pathological
conferences must be held on a regular basis.
7) The hospitals should have appropriate facilities for
autopsy.

The JSCP pre-certified hospitals will not keep a full time clinical laboratory physician certified by the JSCP, but must meet all other requirements for the JSCP-certified hospitals . However, JSCP-pre certified hospitals must have a full-time physician who is a member of the JSCP for at least five years, and possesses appropriate leadership in general clinical pathology(laboratory medicine) as an educational supervisor.

Special training facilities designated by the JSCP shall be the other medical facilities than the JSCP certified and pre-certified hospitals, health check-up centers , independent clinical laboratories, research institutes and other facilities related to clinical laboratory medicine . These facilities should be able to cover an insufficiency of training curriculum for clinical laboratory physicians and meet the following requirements.
1) The clinical laboratory physician certified by the JSCP
shall work as the person in charge of education,
including a part-time position.
2) The facility should stipulate an educational curriculum
for post-graduate clinical pathology training .
3) The facility should have personnel, facilities, medical
instruments, and library, sufficient for education.

The term of validity for certified facilities will be for two years. The JSCP shall certify the renewal with application. If a hospital is certified for more than two consecutive times without problem, the term of validity may be extended to five years, as long as there are no changes in the requirements for certification.

The certification shall be cancelled if the requirements are not met.

The Board of Directors shall approve the revision of this regulation.
Bylaw of Commission on Certification of
Clinical Laboratory Physicians

Objective:
The Commission's objective is to deliberate on the regulation for certified clinical laboratory physicians designated by the JSCP and all matters regarding its operation.

Officers:
The Commission shall consist of several members and the term of office as a member is two years. The President of the JSCP entrusts commission members and holds the position of chairman concurrently.

Committee:
(1) The Commission has the following Committees.
1. Committee on Qualification for Examination
/Recertification
2. Committee on Certification of Training
Centers/Supervisors
3. Examination Committee
4. Other Committees which are necessary for the
operation of a regulation for certified clinical
laboratory physicians
(2) Membership of the commission shall consist of the
chairperson of each committee.

Meeting:
The chairperson will convene a meeting no less than once a year.
Bylaw of Committee on Certification of Training Centers/Supervisors

Objective:
The Committee's objective is to examine and discuss the necessary matters regarding the certification of training centers/supervisors, specified in the regulation for certified clinical laboratory physicians prescribed by the JSCP.

Members of Committee:
The Committee members consist of one or more elected members from each regional branch of the JSCP, and the term of office as a member is two years. The President of the JSCP will entrust committee members.

Meeting:
The chairperson shall convene a meeting no less than once a year and shall examine an application submitted for their propriety. The resolution of meeting must be reported to the Commission.

Objective:
The Committee's objective is to examine and discuss the qualifications of the applicants for the certification examination, and the qualification for therecertification of certified clinical laboratory physicians and related matters, which are specified in the regulation on certification of certified clinical laboratory physicians prescribed by the JSCP.

Members of Committee:
Several members shall organize the Committee and the term of office as a member is two years. The President of the JSCP shall entrust committee members.

Meeting:
The chairperson shall convene a meeting no less than once a year and a resolution of the meeting shall be reported to the Commission.

Objective:
The Committee's objective is to examine and discuss the necessary matters regarding the plan, contents, and other aspects of the certification examination, which are specified in the regulation on certification of for the certified clinical laboratory physicians prescribed by the JSCP.

Members of Committee:
Several members shall organize the Committee and the term of office as a member is two years. The President of the JSCP shall entrust committee members.

Meeting:
The chairperson shall convene a meeting no less than once a year. A resolution of meeting shall be reported to the Commission and approved by the Commission.

Implementation of certification examination:
Certification examination shall be given to those who meet requirements for an examination, will occur once a year, and will be carried out by Executive Committee for Examination. The Examination Committee shall decide whether to pass or fail the applicant and the Commission shall certify that. The Executive Committee for Examination is designated separately.

Character of examination
The certification examination is a test for qualification, it is not a selective examination. Therefore number of successful candidates will not be set beforehand.

Public announcement for examination date and implementation guidances:
1) Examination for those who meet the requirements will be
held once a year at one place, for the time being. It is
scheduled for two days.
2) Examination date and implementation guidances shall be
noticed six months in advance.

Contents of examination
1) The examination will be written and practical tests.
2) A s a general rule, a written examination shall be
descriptive test on both required and elective subject.
3) The practical test will only be given on the required
subject.
4) Contents of examination will be established under
post-graduate educational curriculum prepared by the
Education Committee.
5) Examination content will not be publicly available.

Implementation method for the examination
1) The Executive Committee for Examination shall be
organized at every test. This committee shall carry out
the examination under its responsibility.
2) The Executive committee chairperson will be nominated at
the Commission. The chairperson of the Commission will
select members of the executive committee among the
councilors. The term of office as an executive committee
member is one year and he/she may be re-appointed.
3) Only the name of the executive committee chairperson
shall be announced publicly in advance, and the names of
other members of executive committee will not.

A pass/fail decision for examination and method of certification
1) There will be a marked, total score for written and
practical tests, which include oral and interview tests.
2) The applicant must pass seven of the required subjects
and all elective subjects. However, if he/she failed two
or less required subjects, then he/she will be allowed to
re-take the examination for the failed subjects within
five years of the first test (Limit four
re-examinations).
The conditions of a re-examination for elective subjects
will be handled the same as required subjects. However,
if he/she takes a different elective subject after
disqualification, he/she may change it on the
re-examination.
3) The Executive Committee will mark results of examination
on a pass/fail decision. The joint meeting for the
Executive Committee and the Examination Committee will
draft a pass/fail decision and the Commission shall
certify it.
